3. Sample Evaluation Forms
Please adapt and modify these forms to fit your current hiring search and department’s needs.
The four evaluation form provided are:
1. “Hiring Committee Set Up Form.”
Use this form to list, rank, and define criteria; identify what constitutes a poor, average, and excellent rating for each criterion; and craft interview questions.
2. “Hiring Committee Rating Rubric Evaluation Form.”
Use this to keep committee members’ ratings focused on the predetermined criteria, the evaluation benchmarks, and the potential range of supporting evidence accepted. Repeat for each criterion. Use consistently for all candidates.
3. “Hiring Committee Candidate Decision Form.”
Use one for each candidate to record why a candidate was or was not selected based on those predetermined criteria.
4. “Faculty Evaluation Form.”
Give this to all faculty involved in the search.
